To make an appointment at the location
of 233 Meadow Avenue in East Peoria, call (309) 694-0606.
Someone will personally talk to you weekdays 8:30 AM until 4:30 PM.
Please avoid calling during lunch hour. |

Be prepared to answer questions on name,
address, dental insurance, insured’s ID number and employer. Have
your calendar ready so an appointment can be made. |

Be prepared to either make your
appointment with the doctor or with the dental hygienist. If you make
the first visit with the doctor, your teeth will not be cleaned that
day. X-rays of all teeth are taken on new patients. |

Please show up fifteen minutes early for
your first visit. You will be asked to fill out forms that ask about
medical history, dental history, symptoms and insurance verification.
Plan on fees of at least $125 for an initial visit if there is no
insurance coverage. |

Evening and Saturday appointments are
made by special arrangement but only after plans for treatment are
made. |

A patient with dental insurance should
have significant coverage toward the cost of an initial dental exam
and a dental cleaning. Dental insurance coverage on X-rays is time
dependant. |

Dr. Nalepka does not participate in PPO
or HMO programs. |

Purpose
of the initial dental visit is primarily to establish rapport and
understanding between patient and doctor. |
